What is the most common context for a genuine accident?

Actually this is a pretty common one actually. This sort of thing peaked around the 60s/70s where girls would dress in short skirts knowing full well they would get too hysterical at the concert not to piss themselves. Some girls actively did it on purpose too, you know the whole sexual revolution mixed with drugs, and as it turns out pissing yourself can feel pretty exciting when you are stoned and horny.

Not sure how common it is today, although many night clubs often have suspiciously wet spots in the carpet when they have dance floors.

Concerts/festivals
It's crowded, the toilet queues are long, there may be drinking, you paid a lot of money for the event and don't want to miss anything, maybe you have waited years to see this band. If you leave, you may not get your spot back, or you may not find your friends again afterwards. I'm sure lots of bladders has been pushed to their limits (and beyond) here. This is a situation where I think many people are actually considering peeing themselves. It could be dark, and the people's focus is on the stage, so you could get away with it. Some will take the risk and end up making it to the toilet after the concert, but some will not make it, and maybe it was worth it.
